SWICA offers structured care programmes (disease management programmes) to people with chronic conditions. In addition to medical care and personal support, the programmes include a treatment plan that is tailored to your needs.
Disease management programmes include medical care, personal support and an individually tailored treatment plan.
The programme includes comprehensive advice on nutrition, exercise and overall wellbeing.
The treating physician remains the patient’s main point of contact.
Depending on the programme, therapists or specialist doctors will also be involved in order to provide professional care with as little stress as possible.
A specially trained medical practice assistant delivers some of the treatment, and is responsible in particular for providing personal support and advice as well as information.
SWICA pays the costs of treatment in accordance with the Swiss Health Insurance Act (KVG). Participation is voluntary.
