As a customer with a Favorit Medpharm, Multichoice or Telmed basic insurance plan, you can, in exceptional circumstances, fulfil your notification requirement by notifying santé24 of a doctor's appointment that has already been arranged or taken place using the form below. Have you not been able to reach us by phone? In order to comply with your reporting obligation as a customer with the Favorit Medpharm, Multichoice or Telmed basic insurance plans, you can by way of exception report a past or scheduled doctor’s visit to santé24 using the following form.

  • When getting treatment during a temporary stay abroad
