As a customer with a Favorit Medpharm, Multichoice or Telmed basic insurance plan, you can, in exceptional circumstances, fulfil your notification requirement by notifying santé24 of a doctor's appointment that has already been arranged or taken place using the form below. Have you not been able to reach us by phone? In order to comply with your reporting obligation as a customer with the Favorit Medpharm, Multichoice or Telmed basic insurance plans, you can by way of exception report a past or scheduled doctor’s visit to santé24 using the following form.
These are the cases in which santé24 does not need to be contacted

- In emergencies
- Appointments with an eye doctor, gynaecologist or paediatrician from the Medpharm directory
- Before receiving treatments in connection with consultations as agreed on with a SWICA partner pharmacy or santé24 if administered by a provider from the SWICA directories
When getting treatment during a temporary stay abroad
Favorit Multichoice
- In emergencies
- Appointments with an eye doctor, gynaecologist or paediatrician if administered by a service provider from the Multichoice directory
- Before receiving treatments in connection with consultations as agreed on with a SWICA partner pharmacy or santé24 if administered by a service provider from the SWICA directories
When getting treatment during a temporary stay abroad
Favorit Telmed
- In emergencies
- Children up to the age of 16 can get treatment from a paediatrician without first consulting santé24
- Appointments with an eye doctor or a gynaecologist
